Office Policies
● Professional fees (examination, refraction, contact lens fitting/evaluation, or any services performed by the doctor) are NOT refundable.
● If your new prescription does not meet your vision needs, contact us within 60 days from that date of your initial visit so that we can make the proper prescription adjustments. Additional fees apply after 60 days.
Frames
● House of Eyes will adjust frames, replace any screws, and/or replace nose pads free of charge when frames are purchased from us.
● If patients choose to place new lenses in an old frame, we cannot be held liable for breaks or damage. As frames age, they may become brittle in ways invisible to the human eye. Patients assume the risk when using old frames. They should also be warned that they assume the risk when we adjust older frames. Patients must sign a disclosure form notifying them of these facts.
● Eyeglasses are custom-made for you based on your unique prescription and measurements that vary based on frame. Because it is designed for you only, there are no refunds on any purchased eyewear (including lenses & frames). Lens orders are started as soon as possible to ensure quick turn-around time. Once the lab starts making your lenses, we are unable to cancel the order.
● All sales that are non-prescription sunglasses are final and non-refundable. Patients have 7 days from the date of purchase to exchange a sunglass frame for store credit only. Exchanged frames must be in a re-sellable condition with no damage or scratches in the original packaging.
Lenses
● Lens redo’s and re-refractions will be granted, free of charge, within 30 days IF the lenses were made by House of Eyes under the following conditions: a) RX change b) Optical change [i.e. PD and/or seg height changes] c) Lab Error.
● If lenses were made by an outside office, we will measure the prescription in the new glasses to verify accuracy. If a patient requests a doctor to re-refract they will be charged a refraction fee.
● Lens manufacturer warranties vary as follows, with dates commencing from the date the order was paid for:
○ Zeiss anti-reflective treatments and Lenses have a 1-year warranty that can be used once during that year free of charge.
○ Standard anti-reflective coatings and standard lenses have no warranty.
● Cancellation Policy: In the event you wish to cancel your order it must be done by the close of business on the day of order to receive a full refund. All costs incurred once a prescription order has been started at the lab whether or not completed will be the customer’s responsibility & therefore will not be eligible for a full refund.
Contact Lenses
● With regard to the sale of non-specialty soft contacts lenses, any non-expired, unopened & unmarked boxes may be returned for credit, within 30 days if there has been a change to your prescription. All sales of specialty gas permeable (i.e., RGP) and hybrid (i.e., containing both rigid and soft components) contact lenses are final.
